FGS Form: Graduate Student Library Extension Request

This form is for graduate programs to request an extension of library privileges for a withdrawn graduate student, who has completed all milestones except having written and defended the thesis / dissertation.

The library extension has a cost, and must be sponsored by a faculty member.

Note that all requests are one (1) year in duration. COST is $30 PER YEAR.

If the extension is approved, FGS will process the request with UIT and the YUCard office. If the extension is not approved, FGS staff will contact the person who submitted the form to update them.
